NMC directs medical colleges to maintain patient records

The National Medical Commission (NMC) has issued a circular directing medical college deans to ensure that their hospitals maintain proper patient records under the Ayushman Bharat Health Account (ABHA) ID system. The circular states that patient verification through ABHA IDs will also be part of the assessment process for medical colleges.According to the directive, patient records must include the signatures of the unit faculty and the senior resident doctor.The NMC had earlier issued a circular in 2024 mandating all medical college hospitals to generate ABHA IDs for patients coming in for both emergency medical care and outpatient department (OPD) services. Both indoor and outdoor patients are required to be registered under the ABHA ID system.In the latest circular, the NMC has emphasised that all colleges must maintain accurate and complete patient records. If any records are found to be fake or manipulated, strict action will be taken against the concerned faculty members.The circular also clearly mentions that in case of technical issues preventing the generation of an ABHA ID, hospitals must still provide treatment. Denial of medical care under such circumstances is not permissible.The NMC further stated that while a shortfall of up to 10% in ABHA ID registrations may be tolerated, anything below that threshold will not be acceptable.
